Ages 16+ till 22

Our SIL Program takes clients from the ages of 16 till the age of 22.  In this program it is mandatory for the youth to participate in a progressive day program. This program may be full time school or work or a combination of both. The program plan/goal will be selected in consideration of the youth’s wants, needs and capabilities. Required support will be based on the individual needs and CFH can provide Life Skills learning to fit the client’s schedules after their education program or their work hours.

This program is for youth that are almost ready for the responsibilities that come with independence and adulthood and only require minimal supervision/life skills of usually no more than 2 hours daily – 12 hours weekly (extra staffing hours can be negotiated as needed) with the focus remaining on guidance and training in the areas of personal wellness, nutrition, finance, interpersonal skills, goal planning, time management, home maintenance, making healthy choices, problem solving and self-awareness.

This is accomplished with staff taking the youth shopping and budgeting for groceries and aiding schoolwork, menu planning, cooking skills, bussing schedules, resume writing, job hunting and selection of appropriate resources along with referrals regarding the community resources available to the youth.

In cooperation with the youth and their caseworker the program provides for the development of Individual Service Goals according to his/her Strengths, Needs, Abilities, and Preferences which will include interaction with family and community of origin as agreed to with the case worker during case conference.  Recreational and Cultural participation are strongly supported. Changes For Hope will apply for Leasure Access Pass for all Clients and will help Clients to obtain an Alberta ID and Bank Account and school enrolment if needed. 

The youth’s progress in their goals will be monitored and evaluated on an ongoing basis and the youth’s personal goals are reviewed quarterly.

The home is fully furnished and is complete with Bed, Dresser, Dining room table and chairs, sofa and coffee table, All Appliances, Household Goods such as dishes, pots and pans, utensils, bedding, towels and drapes, Smart TV, Computer, Internet, and Landline House Phone.
